  • 4600 Letter designating an IPM Provider or left blank
  • Patient Demographics (Name, DOB, Gender, Address, Phone, Email, Interpreter Needed)
  • Date of Injury
  • Insurance Information & Claim Number or EAMS ADJ#
  • Employer Information
If available please also send:
  • MPN Name & MPN Number
  • Adjuster and Contact Number
  • Authorized Body Parts
  • Medical Records If Available
